Characteristics of ulcer healing: Venous leg ulcers are a chronic disease characterized by loss of skin integrity. It affects about 1 % of the population. The defect involves the skin and subcutaneous tissues to a varying depth. Venous leg ulcers may develop from arterial causes in patients with advanced stages of lower extremity ischaemia due to atherosclerosis. In most cases, however, ulceration occurs due to long-term venous insufficiency of the lower extremities. Affected individuals have an impaired supply of oxygen and nutrients to the legs, which itself reduces the ability to heal wounds. Risk factors include hereditary predispositions to the loss of elasticity in the blood vessel walls, varicose veins, older age, inflammation in the deep venous system, diabetes, a sedentary job or long periods of standing, lack of physical activity, obesity, as well as a greater number of pregnancies.
Treatment of venous leg ulcers is aimed at eliminating the causes that led to the disease or at least compensating for the underlying and associated diseases, but damage to the vascular system may be so great that it requires amputation. An ulcer is a gateway for infection, and can be complicated by the development of erysipelas.
Restoration of the proper functioning of the venous system in the lower extremities is essential for successful treatment and healing of venous leg ulcers, and may require bandages, exercise, weight reduction, external supportive treatment, removal of necrotic tissue, topical antibiotics, treatment to stimulate healing, special dressings (moist wound care), protection of the surrounding healthy skin, and in the case of surgical treatment, where the base of the ulcer must be pre-treated and prepared for optimal healing of the graft, systemic treatment, vasoprotective agents or vasodilators are appropriate, and suitable rehabilitation therapy is also required.
